Where to start

Pick the task in front of you.

Before an appointment, open the question card. After a conversation, open the instruction template. To prepare for the whole visit, open the visit kit.

Choose paper or screen.

Print the PDF to write by hand, type into the PDF on screen, or save the DOCX to change the wording. File Access Help covers printing at actual size, A4, large print and editing.

Fill in one thing first.

Write one question or one instruction. You do not need to complete every field at once.

Keep it with your paperwork.

Store completed pages in one private place you will remember for the next contact step.

Look inside before you choose

Your notes stay with you.

Keep completed pages on your device or with your own paperwork. Nothing you write in the files is sent to us. Do not send photos, medication lists or completed health notes through any form on this site.

Caregivers should have permission before recording or sharing another person’s information.

Helpful questions

Do I need to buy anything?

No. Every resource here is free and useful on its own.

Do I have to join a mailing list?

Not an ongoing one. The downloads ask for your first name and email. We send a short email; reply VERIFY to it and we email you the link, then 4 short tip emails over 11 days, and then the series ends. Nothing is for sale, and every email has an unsubscribe link at the bottom. The prompt bank is free to browse; the pack builder comes with the same sign-up.

Do these resources give medical advice?

No. They help you organize your questions and records. Ask a qualified health professional about individual health concerns.

Which format should I choose?

Choose the PDF if you want to print and write by hand. Choose the DOCX if you want to type and save a private copy on your device. You can use both.

Update, October 20, 2026: the free downloads now ask for a first name and email address, and this page has been updated to say so.
